I missed a midterm examination due to illness. What do I do?

A UOIT Medical Statement must be filled out by a licensed practitioner within 24 hours of the missed midterm examination. The completed Medical Statement must be submitted to the Faculty of Science Academic Advising office within three working days to be considered. If the Medical Statement is approved, the midterm examination will be re-weighted to the final examination. The Medical Statement form can be accessed through the student forms page.

I missed a final examination due to illness. What do I do?

Two forms must be completed: a UOIT Medical Statement and an Application for Deferred Final Examinations. The Medical Statement must be filled out by a licensed practitioner within 24 hours of the missed final examination. Both forms must be submitted to the Office of the Registrar within three working days to be considered. If the documents are approved, a new examination will be scheduled during the deferred examination dates. I missed a laboratory/tutorial session or an assignment/report deadline due to illness. What do I do?

A UOIT Medical Statement must be filled out by a licensed practitioner within 24 hours of the missed work. To be considered, the completed Medical Statement must be submitted directly to the Instructor or Senior Laboratory Instructor within three business days. Consideration regarding the missed work is at the discretion of the Instructor of Senior Laboratory Instructor. Please refer to your course syllabus for further instructions. I missed a midterm due to extenuating circumstances. What do I do?

Please write a formal letter explaining the circumstances surrounding the missed midterm and complete the Academic Consideration Form. This should be submitted to the Academic Advising office within three business days of the missed exam. In addition to the letter, please include any supporting documentation with your submission. The Academic Consideration form can be accessed through the student forms page.

I missed a final examination due to extenuating circumstances. What do I do?

Please write a formal letter explaining the circumstances surrounding the missed final examination and complete the Academic Consideration Form. The Academic Consideration form must be filled out and submitted to the Academic Advising office. For midterm examinations, the forms must be submitted no later than seven days prior to the date of the midterm examination. For final examinations, the form must be submitted three weeks prior to the start of the final examination period. The Academic Consideration form can be accessed through the student forms page.
